ADMISSION REQUIREMENTS
The following forms must be
completed prior to admission and
should be updated whenever needed.
This is a Board of Health
Requirement.
- Your child’s Birth Certificate or
Passport, or some official
documentation showing your
child’s proof of age.
- Your child’s medical form — all medical forms must be on file in our school office before your child's first day of school.
- Immunization Records
*Children may be registered only after a personal interview with the Director.*
REGISTRATION FEE
A $80 non-refundable registration fee for each child is required with your application.
